Pursuant to the Personal Data Protection Act of 10 May 2018 (Journal of Laws 2018, item 1000) and in agreement with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation),
we would like to inform you that:

The basis for processing of your personal data is Article 6(a), (b), (c), (f) of the Regulations. Providing your personal data is a statutory and/or contractual requirement. The consequence of not providing the data may be the inability to conclude a contract, as well as the lack of contact. You have the right to access your data and the right to rectify it, limit processing, in justified cases to delete (“the right to be forgotten”), the right to object to processing, the right to transfer data. According to Art. 13 of the Regulation, you also have the right to withdraw your consent to the processing of personal data at any time without affecting the lawfulness of the processing which was carried out on the basis of consent before its withdrawal.
You have the right to lodge a complaint to the President of the Personal Data Protection Office if you believe that the processing of your personal data violates the provisions of the Regulation and the Act. The expected recipients of your personal data are: public institutions authorized to receive and process your personal data in accordance with the applicable law, entities related by capital or personally to the company, other entities processing data on behalf of the Administrator (based on separate entrustment agreements), as well as employees and associates of the company or these entities – to the extent necessary to achieve the purposes of data processing.
Your data will be kept for the period necessary to achieve the indicated purposes of processing, including the duration of the contact of the parties, the possible validity of the contract, after its termination or after its termination – for the period of limitation of claims, and if it concerns for the time necessary under separate provisions on storage of accounting and accounting documentation, as well as provisions regarding the archiving of documents.
Your data will not be transferred outside the EEA, i.e. the European Economic Area, unless the contract between the parties contains different provisions. Your personal data will not be processed in an automated manner (including in the form of profiling) in such a way that as a result of such automated processing, any decisions could be made, other legal effects would be caused or it would have a significant impact on your rights.
